1. Read "My Birth Stories" to get a feel for the information I will be requiring for my book.
2. Follow the "Birth Story Template" to organize your thoughts and write down your personal birth stories.
3. Copy and Paste the "Consent Form" to a word document.
4. Print out the form, sign and mail the original to my post office box found under "Contact Me", along with your written birth stories.
